**Job Summary:** The Scheduler is responsible for the coordination of scheduling Private Duty services in collaboration with the Private Duty Director, distributes patient information to assigned staff, Enters clinical staff schedules timely and accurately into the computer daily.

**Essential Functions:**

Provides coverage for phones through main Private Duty line. Assists in insuring all calls into department are answered timely.

Schedules staff to patient visits according to area, skill level, and continuity of patient care and staff availability.

Enters and attends patient schedules for all disciplines into EHR on a daily basis.

Generates schedules for each discipline and verifies schedules are accurate with assigned staff.

Utilizes available staff in most efficient manner to insure adequate staff coverage to meet safe and timely patient care delivery.

Notifies team members of discharged, expired, hospitalized and changes in patients immediately

Able to accurately explain Private Duty Services, costs and process to the customers satisfaction

Is prepared to satisfy the caller's need as expeditiously as possible with appropriate follow through

Reviews aide note/timecard for content, accuracy and completeness. Presents discrepancies and concerns to Director.

Reviews SN notes and timecards for accuracy and completeness. Presents discrepancies and concerns to RN or Director.

Responds to patient/customer calls in a customer service value based manner.

Assists with coordination of ordering office/medical supplies

Coordinates and directs delivery of patient care.

Maintains patient confidentiality and network confidentiality in all areas.

Demonstrates knowledge of human resource principles.

Is knowledgeable of network policies and procedures including safety and disaster preparedness.

Annually completes Compliance Education and understands applicable policies.

Fulfills additional duties as assigned.

**Additional Responsibilities:**

Completes QI's as appropriate on all patient complaints, risks, hospitalizations, infections, falls etc

Participates in departmental meetings; assists in the successful implementation of new/changed processes

Assists with orientation of new field staff as requested.

On-Call rotation up to 2 weeks/weekends per month and holiday rotation

Completes compliance education and understands all applicable policies.

Performs other duties as assigned.

**Minimum Position Qualifications:**

Education: High School diploma or equivalent

Experience: One year of experience in a health care setting or related scheduling field

**Preferred Position Qualifications:**

Home health experience, data entry skills, scheduling experience, excellent customer service and problem solving skills. Able to multitask and prioritize.
